Analyze data and trends to identify areas for improvement and track progress towards population health goals. Lead and manage a team of population health professionals to ensure effective and efficient execution of initiatives. Communicate and build relationships with key stakeholders, including healthcare providers, community organizations, and government agencies. Ensure compliance with all regulatory requirements related to population health management. Stay updated on industry trends and best practices in population health management and incorporate them into the organization's strategies. Develop and manage a budget for population health initiatives, ensuring resources are allocated effectively. Advocate for population health initiatives within the organization and externally to promote awareness and support. Collaborate with cross-functional teams to integrate population health strategies into existing programs and services. Evaluate the success of population health initiatives and make recommendations for improvement. Develop and deliver presentations and trainings related to population health management. Maintain accurate and detailed records and reports on population health initiatives and outcomes.
